         (Profile Information is Optional)

An individual profile will tell a little about the individual. Since we are profiling people from a broad range of ages, some items may not be relevant to the person you are submitting information for. Please give us as much information as possible.

1. Picture of soldier.
   - This should be a good quality digital or photograph
    - For soldiers, prior to 1990, we would like to have
      a picture of the soldier in uniform and a current photo.

2. Parents of individual.
     {example}
   - Jim and Sally Smith of Utopia
    - Jim Smith of Utopia and Sally Jones of Vanderpool
    - explain any other situations.

3. Grand-Parents of individual.
   - same info as for parents

4. Spouse of Individual.
   - If individual qualifies to be listed because of their spouse
     being from the Utopia-Vanderpool area; then we need
     the same information for the spouse from #2 and #3 above.

5. High School information.
   - Year and name of HS  when graduated,
    - If not graduated, then years of attendance.
    - HS achievements (Nat Honor Society, etc)

6. College Information (if applicable).
    - Year and name of college where graduated
    - If not graduated, then years of attendance
    - Major or Degree in what field
    - College achievements (ROTC, etc)

7. Military Service.
  - Awards and Honors
   - Service Ribbons
   - Service History (duty stations or ships)

8. Local Church.
   - Individual member of any local church
     (Utopia or Vanderpool area)

9. Additional Information.
   - any additional information you want to tell about the individual
   - we will use what we can to help honor the individual

10. Submitter's Information.
   - your name and phone number for follow-up contact, if needed.
